* Please note that the fee for evaluations is $150.00. If you schedule for surgery, the $150 fee will be put towarads the cost of your procedure.

** Screenings are free. If you are found to be a candidate following your screening, you may proceed for evaluation. You may also skip the screening process and move straight to the evaulation process if you so choose.

Both the Topeka and Manhattan offices are open Monday through Friday to make appointments, answer questions and handle prescription refill requests.
