* Please note that the fee for a LASIK evaluation is $150.00. If you schedule for surgery, the $150 fee will be put toward the cost of your procedure.

 

** LASIK Screenings are free. If you are found to be a candidate following your screening, you may proceed for evaluation. You may also skip the screening process and move straight to the evaluation.
